Frequently Asked Questions

Yes. We are currently accepting new patients at our Snellville office and for house call visits.

You may be eligible for house call visits if one or more of the following applies:

  • You currently live in an assisted living facility or personal home.
  • You require the assistance of another person to leave home.
  • You require the use of a cane, walker, or wheelchair.
  • You must exert a high level of effort to get in an automobile.
  • Your visits away from home are infrequent and of short duration.

You do not need a referral to use our house call services.

Our providers will sometimes order lab tests, x-rays, and other services to fully evaluate your medical situation. Very often, these services can be done in your place of residence if you are homebound. The charge for these tests and procedures will be billed by the companies performing them. Imaging studies that require large equipment, such as CTs or MRIs, must be done at a local imaging center or hospital outpatient department.

We accept Medicare (including most Medicare Advantage plans), Medicaid, Wellcare, Peach State Medicare, Peach State Medicaid, Allwell, Ambetter, CareSource Medicaid, CareSource Marketplace/ACA, Amerigroup, Humana, Aetna (including Exchange plans), United Healthcare Commercial & Medicare Advantage Plans, Coventry, First Health, Cigna, Cigna HealthSpring, UPMC, BCBS, Clover Health Plans, Sonder Health Plans, Oscar Health Plans, and Georgia Health Advantage. We also accept most supplemental plans. We only bill the patient if there’s an unpaid balance which is not covered by their insurance. Medicare Part B covers 80% of our services, after the annual deductible is satisfied. Many secondary plans pay the 20% balance. We also accept self-pay.

Yes. Medicare pays 80% of the allowable amount for housecall/PCH/ALF visits. The remaining 20% balance is carried over to the supplemental insurance or patient. Visits for patients with a Medicare Advantage plan will be subject to that particular plan’s benefits- typically the same as the plan’s benefits for a PCP office visit.

Depending on the type of form, there is usually a fee for having forms filled out. Below is some information and the fees associated with some of the more common forms:

  1. Blank forms will not be accepted. Personal information must be completed.
  2. Outstanding balances need to be paid prior to forms being filled out.
  3. Many forms require a current exam prior to being completed.

The charge for review and completion of medical forms is as follows:

  • FMLA Paperwork- $25.00
  • Physician’s Evaluation for Assisted Living (patient must have current TB skin test or chest x-ray results for form to be completed) – $25.00
  • Handicapped Parking Affidavit (includes Notary fee) – $5.00
  • VA form: Examination for Housebound Status/Aid & Attendance (21-2680) – $25.00
  • Physician’s form for Nursing Home or Intermediate Care (DMA-6), including PSARR form when required- $25.00
  • Citizen and Immigration: Medical Certification for Disability Exceptions (N-678) – $20.00
  • Medical Records Request (in accordance with and established by the Georgia Department of Community Health) –
  • Administration/Retrieval: $5.00 PLUS
  • Per page for pages 1-20: $0.97
  • Per page for pages 21-100: $0.83
  • Per page for pages over 100: $0.66
  • Notary fee (when required): $2.00

What Is a Nurse Practitioner?

A nurse practitioner (NP) is a registered nurse (RN) who has completed advanced coursework and clinical education (a minimum of a master’s degree) in the diagnosis and management of medical conditions, including chronic illnesses. The core philosophy of the NP field is individualized care. Nurse Practitioners focus on patients’ conditions as well as the effects of illness on the lives of the patients and their families. If prescriptions are needed, NPs in the state of Georgia are able to prescribe and manage medications.

Patricia Gray-Smith

APRN BC FNP/ANP

Nurse Practitioner Patricia Gray-Smith has over twenty-five years of nursing experience. She received her Master’s degree in Nursing and her Post Master’s Certificate in Family Health from Stony Brook University of New York. Patricia provides both primary care and geriatric psychiatry to her patients and is committed to caring for the most vulnerable populations. Her passion lies with older adults, especially those with dementia. And as such, her goal is to provide health care in the patient’s own familiar setting.

Paul E. Allen, MD

Horizons Healthcare Physician

Dr. Paul Allen is a physician specializing in internal medicine. Additionally, Dr. Allen has over 25 years of experience in the emergency room. Dr. Allen received his medical degree from State University of New York and completed his residencies at Harlem Hospital Center and Bergen Pines County Hospital. He sees patients ages 16 and up for primary care in our Snellville office. He also performs sports physicals for patients 13 and up.

Nicole Moncrieffe

APRN BC FNP

Nicole Moncrieffe is a nurse practitioner who has extensive nursing experience which includes care of adults across their lifespan. Her focus includes physical/wellness exams and assessments, acute and minor care, and treatment of chronic health issues. One of her passions is working with older adults to help manage their chronic diseases and to educate both the patients and their caregivers regarding their specific healthcare needs. Nicole received her Post Master’s Certification in Family Health from the University of Arizona.

Marcia Oudkerk

APRN BC FNP

Nurse Practitioner Marcia Oudkerk began her nursing career in 2001. She received her Master’s degree in Nursing in 2018 and is certified by ANCC (American Nurses Credentialing Center). Marcia provides comprehensive chronic disease management and community based primary care visits. Additionally, she addresses behavioral disturbances that often accompany dementia. Her goal is to provide regularly scheduled medical care to her elderly patients in order to avoid emergency care or hospitalization when possible. As a Certified Case Manager, Marcia is able to collaborate with community health partners to ensure the best possible health outcome for her patients. She sees patients where they live to ensure quality and continuity of care.
