Horizons Healthcare has been in business since 2006. Our goal is to provide comprehensive care in our Snellville office or the patient’s own living environment. Our nurse practitioners manage acute and chronic medical conditions through comprehensive history taking, physical exams, and the ordering of diagnostic tests and medical treatments. Our nurse practitioners also specialize in geriatric psychiatry, assessing and treating mood disorders, confusion, and behavioral disturbances that often accompany dementia.
A nurse practitioner (NP) is a registered nurse (RN) who has completed advanced coursework and clinical education (a minimum of a master’s degree) in the diagnosis and management of medical conditions, including chronic illnesses. The core philosophy of the NP field is individualized care. Nurse Practitioners focus on patients’ conditions as well as the effects of illness on the lives of the patients and their families. If prescriptions are needed, NPs in the state of Georgia are able to prescribe and manage medications.

Nurse Practitioner Patricia Gray-Smith has over thirty years of nursing experience. She received her Master’s degree in Nursing and her Post Master’s Certificate in Family Health from Stony Brook University of New York. Patricia is certified by AANP (American Academy of Nurse Practitioners). During her time working as a Hospitalist at Beth Israel Hospital in New York, Patricia became aware of the gap in transitional and on-going care when the patients were discharged back home. With that need in mind, Horizons Healthcare was established in 2006. Patricia provides both primary care and geriatric psychiatry to her patients and is committed to caring for the most vulnerable populations. Her passion lies with older adults, especially those with dementia. And as such, her goal is to provide healthcare in the patient’s own familiar setting. Today she serves patients both in our Snellville office and in the community where her patients reside.
Dr. Paul Allen is a physician specializing in internal medicine. Additionally, Dr. Allen has over 25 years of experience in the emergency room. Dr. Allen received his medical degree from State University of New York and completed his residencies at Harlem Hospital Center and Bergen Pines County Hospital. Patients benefit from his extensive medical expertise and ongoing quest for knowledge. He sees patients ages 18 and up for primary care in our Snellville office.
Nurse Practitioner Nicole Moncrieffe has extensive nursing experience which includes providing care for adults across their lifespan. She received her Master’s degree in Healthcare Administration and her Post Master’s Certification in Family Health from the University of Arizona. She is certified by AANP (American Academy of Nurse Practitioners). Her focus includes physical/wellness exams and assessments, acute and minor care, and treatment of chronic health issues. One of her passions is working with older adults to help manage their chronic diseases and to educate both the patients and their caregivers regarding their specific healthcare needs. She enjoys the interactions she has with her patients and believes the personal connections contribute toward the overall relationship with each patient. Nicole is known for her warm smile and sunny disposition.
Nurse Practitioner Marcia Oudkerk began her nursing career in 2001. She received her Master’s degree in Nursing in 2018 and is certified by ANCC (American Nurses Credentialing Center). Marcia provides comprehensive chronic disease management and community based primary care visits. Additionally, she addresses behavioral disturbances that often accompany dementia. Her goal is to provide regularly scheduled medical care to her elderly patients in order to avoid emergency care or hospitalization when possible. As a Certified Case Manager, Marcia is able to collaborate with community health partners to ensure the best possible health outcome for her patients. She sees patients where they live to ensure quality and continuity of care.
