What Is a Nurse Practitioner?

A nurse practitioner (NP) is a registered nurse (RN) who has completed advanced coursework and clinical education (a minimum of a master’s degree) in the diagnosis and management of medical conditions, including chronic illnesses. The core philosophy of the NP field is individualized care. Nurse Practitioners focus on patients’ conditions as well as the effects of illness on the lives of the patients and their families. If prescriptions are needed, NPs in the state of Georgia are able to prescribe and manage medications.

Patricia Gray-Smith

APRN BC FNP/ANP

Nurse Practitioner Patricia Gray-Smith has over twenty-five years of nursing experience. She received her Master’s degree in Nursing and her Post Master’s Certificate in Family Health from Stony Brook University of New York. Patricia provides both primary care and geriatric psychiatry to her patients and is committed to caring for the most vulnerable populations. Her passion lies with older adults, especially those with dementia. And as such, her goal is to provide health care in the patient’s own familiar setting.

Paul E. Allen, MD

Horizons Healthcare Physician

Dr. Paul Allen is a physician specializing in internal medicine. Additionally, Dr. Allen has over 25 years of experience in the emergency room. Dr. Allen received his medical degree from State University of New York and completed his residencies at Harlem Hospital Center and Bergen Pines County Hospital. He sees patients ages 16 and up for primary care in our Snellville office. He also performs sports physicals for patients 13 and up.

Nicole Moncrieffe

APRN BC FNP

Nicole Moncrieffe is a nurse practitioner who has extensive nursing experience which includes care of adults across their lifespan. Her focus includes physical/wellness exams and assessments, acute and minor care, and treatment of chronic health issues. One of her passions is working with older adults to help manage their chronic diseases and to educate both the patients and their caregivers regarding their specific healthcare needs. Nicole received her Post Master’s Certification in Family Health from the University of Arizona.

Marcia Oudkerk

APRN BC FNP

Nurse Practitioner Marcia Oudkerk began her nursing career in 2001. She received her Master’s degree in Nursing in 2018 and is certified by ANCC (American Nurses Credentialing Center). Marcia provides comprehensive chronic disease management and community based primary care visits. Additionally, she addresses behavioral disturbances that often accompany dementia. Her goal is to provide regularly scheduled medical care to her elderly patients in order to avoid emergency care or hospitalization when possible. As a Certified Case Manager, Marcia is able to collaborate with community health partners to ensure the best possible health outcome for her patients. She sees patients where they live to ensure quality and continuity of care.
